The mPower project aims to support adults facing a range of issues in their lives that may affect their health and their ability to manage in their home. mPower helps people to take the steps needed to live well, safely and independently in their own homes. It does this by linking them with services in their community and by helping people use technology to look after their own health and care.

Referrals to the mPower service can be done through GP’s and other health care professionals as well as self-referral. Once the mPower team receives a referral, the Community Navigator will plan and undertake a home visit to have a confidential conversation about what really matters to the service user.

What is a Community Navigator? The Community Navigator is a member of staff who will work with service users to develop a Wellbeing Plan. This sets out things that can be done to link the service user up with activities in the local community. It also looks at how to use technology (like mobile phone apps) to increase support for health and wellbeing.
